Creatine: What the Evidence Actually Shows for Muscle and Brain
A grounded look at creatine research: strength gains, emerging cognitive findings, the standard 3-5g dose, and why the water-retention and kidney myths do not hold up.
Creatine has been studied for decades, which is unusual for a supplement — most trends come and go before the research catches up. That long track record is part of why it keeps getting recommended by clinicians and coaches who are otherwise cautious about supplements. Still, “well-studied” is not the same as “solves everything,” so it helps to separate what the evidence actually supports from what marketing has bolted on top of it.
What creatine does in the body
Creatine is a compound your body makes on its own, in small amounts, mostly stored in skeletal muscle. It plays a role in regenerating ATP — the immediate energy currency muscles use for short, forceful efforts, like the last few reps of a heavy set or a hard sprint. Supplementing raises the pool of stored creatine (as phosphocreatine) above what diet alone typically provides, particularly for people who eat little or no meat, since food is the main dietary source.
More available phosphocreatine may mean muscles can regenerate ATP a bit faster during brief, intense efforts. That is the mechanism behind most of the benefits people report — it is not a stimulant, and it does not work like a pre-workout jolt.
The strength and power evidence
This is the area with the deepest research base. Across many trials in resistance-trained and untrained adults, creatine supplementation combined with resistance training has generally been associated with:
- Greater gains in strength and power output compared to training alone
- Modest increases in lean mass, likely a mix of muscle water content and actual tissue growth over time
- Possible improvements in high-intensity, repeated-effort performance — think sets of sprints or repeated jumps, more than steady endurance work
The effect sizes vary by study, population, and training program, and creatine is not a substitute for progressive training or adequate protein intake. It appears to work best as a modest amplifier of a training stimulus that is already there, not as a driver of results on its own.
What about the brain?
Interest in creatine and cognition is newer and the evidence is less settled than the strength research. Creatine is also used by brain tissue for energy metabolism, which is the rationale for looking at cognitive effects. Some research has explored whether supplementation might support aspects of memory or mental performance, particularly under conditions of fatigue, sleep deprivation, or stress, when the brain’s energy demands may be harder to meet.
This is a genuinely active and evolving area, and results across studies have been mixed — some show modest benefits under specific conditions, others show little difference. It would be overstating things to call creatine a cognitive enhancer at this point. It is fair to say the question is scientifically interesting and worth watching, without treating it as settled.
Standard dosing: what the research actually uses
Most of the strength and body-composition research relies on a fairly simple, consistent protocol:
- Maintenance dose: around 3-5 grams per day, taken consistently
- Optional loading phase: a higher dose (often split across the day) for roughly a week, used to saturate muscle stores faster
- Timing: appears to matter less than consistency — taking it around a workout is a reasonable habit, but there is no strong evidence that a precise time of day is required
- Form: creatine monohydrate is the form with by far the most research behind it; other forms are marketed as “improved,” but the evidence supporting a meaningful advantage over monohydrate is thin
Skipping the loading phase and simply taking 3-5g daily still gets most people to saturated muscle stores — it just takes a few weeks longer to get there.
Debunking two common myths
“Creatine makes you retain water and look bloated.” Creatine does pull some water into muscle cells, which is a real, measurable effect and can show up as a small bump in body weight, especially in the first week or two. That is intracellular water — inside the muscle — which is different from the kind of puffy, subcutaneous water retention people tend to picture. For most healthy users, this effect is modest and levels off rather than continuing to increase.
“Creatine damages your kidneys.” This concern likely traces back to the fact that creatine is broken down into creatinine, a marker doctors use to estimate kidney function. Supplementing can raise creatinine slightly, which may look concerning on a lab test even when kidney function itself is unchanged. In people with healthy kidneys, the available research at commonly used doses has not demonstrated kidney damage. That said, this research is largely conducted in healthy populations, so it does not directly answer the question for people who already have reduced kidney function or kidney disease.
Who should still check with a doctor first
Creatine is one of the more reassuring supplements from a safety standpoint in generally healthy adults, but “generally healthy” is doing real work in that sentence. It’s worth a conversation with a clinician before starting if you:
- Have existing kidney disease or significantly reduced kidney function
- Are pregnant or breastfeeding, where research is limited
- Take medications that also affect kidney function or fluid balance
- Have a diagnosed heart or liver condition
- Are under 18, where research is more limited and any use should be medically supervised
For most other healthy adults, creatine monohydrate at 3-5g daily is among the more conservative, well-characterized supplement choices available — which is exactly why it has held up as long as it has.
